Eyeglasses that have been given to children in our optical clinics can be replaced if they have been lost or broken for up to 2 years after they were first given. A child should contact their school DOE Representative who must complete all of the information requested below. A representative from our ministry will be in contact with the DOE Representative to make arrangements for replacement.
